We require pet parents to complete certain forms before their pet’s walk/visit/stay. You can download the required forms and email once completed to infopawsabilities@gmail.com

Boarding Agreement
This form outlines the terms and conditions governing your pet’s stay with us. Please read it carefully, sign where indicated and return it to us in advance; otherwise you may bring it with you at check-in.
Pet Assessment Form
This form details your pet’s history, health and behaviours so we can customize our care and attention during their stay. Must be completed at check-in and updated annually.
Medication Form
This form outlines the medication, vitamins or supplements your pet needs to take during their stay. Call ahead to make sure we accept the medication being administered. We do not administer injections. All medication must be in its original packaging and clearly labelled with the type of medication and dosage. Over the counter medications can only be accepted when accompanied by a veterinarian’s dosage recommendation.

Authorization for Release of Vaccinations Form
This form helps you request vaccination information from your veterinarian. Please ask your vet to complete the form and email it to us at infopawsabilities@gmail.com
